Follow these steps to get your RMA:
***Do not submit more than one repair form. Contact repairs@encoder.com for assistance.

  1. Fill out the form below.

  2. Print the form. You will be redirected to a webpage with your form submission, and a copy will be emailed to you. 
    *** If you are not redirected, please refresh the page and your completed form should display.     You can also print the form sent to your email. Print the form, and include it with the encoder you're sending in.

  3. We will contact you with a repair estimate after your encoder has been evaluated.
NOTE: Don't send an encoder in without this form – we need to know who to contact about repairs.
